Complete this form if you are seeking a coordinator's membership for a municipality, county or government agency. Cost is $75 for a municipal/coordinator's membership, and $25 for each additional representative.


Associate/Affiliate Membership
You can designate up to five associate/affiliate members on this form. Cost is $25 for each associate/affiliate member.
